Overall team supervision and leading. Facilitating linkage between HIV testing, diagnosis, care, support and treatment. Initiating Pre-Exposure and Post-Exposure prophylaxis to beneficiaries per eligibility criteria and guidelines. Initiating Antiretroviral Treatment to all positive cases per universal test and treat standards and facilitating index testing to all possible contact or sexual partners of all positive cases. Specimen collection for baseline bloods and sputum for HIV/TB clients, suspects and contacts. Offering all sexual reproductive health services including Cervical Cancer screening and providing contraceptives as per client's choice. Treatment of sexual related infections and diseases and treatment of all contact sexual partners. Tb screening, investigation and index /contact testing of all positive cases. Organising and leading awareness campaigns. Organising all equipment needed by teams and manage the equipment. Ensuring confidentiality and eligible documentation of patient information. Accurate patient record and statistics submission to the Organization and D.o.H as required.
Executing all unsuppressed viral load cases/Lists in the facility according to the facility data system.i.e. TIER.Net. with a goal of ensuring that at least 95% have viral loads suppressed. Taking bloods and to all patient according to their cohorts and overdue lists. Screening of HIV positive clients at every visit for TB (DS-TB and MDR-TB). Lab tracks all pending results including baseline and monitoring to ensure timeous auctioning of abnormal results and treatment effects. Ensuring pre-retrieval of files is done according to ideal clinic and appointment scheduling. Monitoring all patient on chronic medication for side effects and response to treatment. Auditing of all files to identify HIV positive patient that have never started and completed TPT and other prophylactic Therapies. Dispensing antiretroviral drugs according to the guidelines and switching of patient eligible to DTG containing regimen. Providing one stop shop services to all patient coming for chronic medication.ie sexual reproductive health, treating acute sickness and sexual related infections. Facilitating Men’s clinic for retention of all Men in the facility care and providing prostate cancer screening services. Ensuring that every visit is clearly documented and captured by data capturers. Compiling all reports, leading weekly huddle and monthly nerve Centre meetings according to operation Phuthuma programme. Facilitating differentiated models of care for all stable patient on chronic treatment through CCMDD system. Monitor performance quality assurance and performance against target. Lead Quality Improvement Plan (QIP) development. Reviews and discusses team reports. Analyse trends, give feedback to the team, and make recommendations.
Facilitating youth friendly services as a youth champion of the facility. Offering integrated health services to all AGYW/ABYM in the facility i.e. sexual reproductive health, treating sexual infection, acute illnesses and all biomedical services.
